Administration Revises Childhood Vaccine Recommendations

Action: On August 10, the President issued an Executive Order reducing the number of diseases for which immunization is recommended for all children from 18 to 11.¹

• Under the Order, Respiratory Syncytial Virus (RSV), hepatitis A and B, rotavirus, meningococcal disease, influenza, and COVID-19 move to recommendations only for certain high-risk populations, doctor-patient conversations rather than recommendations, or both.
• The Order also calls for the combined measles, mumps, and rubella (MMR) vaccine to be administered as three separate shots once single-disease products become available and “to the maximum extent feasible” for childhood vaccines to be administered at separate medical visits. As separate vaccines have not been available in the US since 2009, manufacturers would need to introduce single-disease products and obtain regulatory approval; they seem reluctant to do so now. The Order also preserves access to combination vaccines.
• The changes widen the divergence between Federal recommendations and those of the American Academy of Pediatrics (AAP), which continues to recommend the former immunization policy.² Some states and private plans increasingly rely on AAP or other sources for guidance.³
• Major national medical and public health organizations have criticized the changes, arguing that no new evidence supports significantly altering the schedule and warning that additional visits could reduce vaccine uptake. They also question the Administration’s reliance on international comparisons because disease patterns and health systems differ across countries.⁴ 
• Implementation remains uncertain, in part because earlier changes to the Federal childhood vaccine schedule have been temporarily blocked by a court.⁵ The Order does not immediately remove vaccines from the market, establish a new dosing schedule, or change state school-entry requirements. A Task Force on Safer Childhood Vaccines⁶ must submit implementation plans to the White House within 90 days.
• What this means for CEOs:
  • Coverage decisions: The Order will not change vaccine coverage immediately. Major private insurers have pledged to maintain existing childhood vaccine coverage through 2027.⁷ Employers, particularly self-funded plans, may likewise maintain broader benefits.
  • Provider planning: Diverging Federal, state, and AAP recommendations will complicate clinical guidance, patient communications, scheduling, billing, and vaccine inventory. Separate vaccination visits could add administrative burdens, while state-by-state differences may further complicate operations for multi-state health systems.
  • Manufacturers: HHS is directed to work with the private sector to develop single-disease options while preserving combination vaccines, requiring manufacturers to weigh development, regulatory approval, production, and distribution decisions.
