This balance is best achieved through a market-driven system, based on private-sector competition and cost-conscious consumer choice.</p> 158 [empty string] <p>Health care is simultaneously a deeply personal issue and a more mundanely economic one. The personal consequences are known to all of us. Economically, many households fear the financial risk of a prolonged illness or serious injury. Health care costs are the most frequent cause of personal bankruptcy. And, from a national perspective, growing health care costs have profound implications for the country&rsquo;s long-term fiscal health.</p> <p>Because of health care&rsquo;s very central role in our personal and economic wellbeing, CED believes strongly that:</p> <ul> <li>Every American should have access to health care.</li> <li>Care should be of high quality.</li> <li>Care should be affordable.</li> </ul> <p>The Patient Protection and Affordable Care Act of 2010 (known informally as the ACA) was a well-intentioned attempt to deal with these issues, but in CED&rsquo;s view, it has fallen short. Further reform is necessary to retain the ACA&rsquo;s positive elements, while simultaneously improving upon it to build a better health care system for all Americans.</p> <p>Our vision for Health Care builds on the ACA&rsquo;s advances by strengthening and broadening the new law&rsquo;s use of those market incentives to drive innovation for higher quality and lower cost, while maintaining an appropriate role for government to facilitate access and ensure that markets work. We believe that this truly would be the achievement of all three objectives of quality, affordability, and access that policymakers have sought for many years. </p> <p>CED believes policymakers have an opportunity to transform the ACA into a system driven by market incentives that decrease costs, improve quality, and promote innovation—while increasing access to coverage. Our research also explores the best path forward for Medicare, which is projected to be the strongest cost driver in the federal budget.</p> <p>CED Members play an instrumental role in forming CED research findings, policy statements and recommendations for reform. Members take an active role in raising awareness about the need for reform through local forums, op-eds, and media appearances. Our members and staff routinely briefs lawmakers about how its recommendations benefit patients, providers, and government.</p><h3><strong><em>CED Policy Recommendations</em></strong></h3> <p>CED has developed recommendations that harness market forces under both the Affordable Care Act and Medicare. </p> <p>Affordable Care Act:</p> <ul> <li>Replace the ACA&rsquo;s complex subsidy mechanism, which puts a heavy compliance burden on and may mislead families with modest incomes and has proved difficult to administer accurately. </li> <li>Restructure the ACA exchange system to align more closely with cohesive geographic health care market areas.</li> <li>Broaden the exchange populations to increase the numbers of enrollees and also the risk diversity, especially in small geographic areas.</li> <li>We would expand the ACA&rsquo;s increase in consumer choice of insurance plans—which is the key to competition and innovation. </li> <li>Remove the ACA&rsquo;s unpopular mandates—and their complex exemptions—to compel the purchase of insurance.</li> <li>Replace the ACA&rsquo;s income-conditioned premium subsidies with a &ldquo;fixed-dollar&rdquo; refundable tax credit, usable only to purchase insurance. </li> <li>Eliminate the unnecessary individual and employer mandates. </li> <li>Reform the tort system, using new data and analysis to formulate rebuttable standards of sound practice. Create specialized expert courts to facilitate more timely and less costly decisions.</li> </ul> <p>Medicare:</p> <ul> <li>Eliminate the Medicare Advantage price benchmark based on traditional Medicare&rsquo;s fee-for-service cost, and provide enrollees with a premium subsidy. </li> <li>Increase the income-conditioning of Part B and Part D premiums, including a temporary Part B premium reduction for lower-middle income seniors. </li> <li>Risk-adjust premium revenue for plans. </li> <li>In rural areas, allow Medicare beneficiaries to enroll in traditional Medicare at no additional out-of-pocket cost, until Medicare Advantage plans meet a minimum threshold of availability.</li> </ul> [empty string] Health Care To address rising health care costs, low-quality care, and inadequate access, CED advances recommendations for market-based and consumer-driven health care reform. To put capitalism on a sustainable path, CED develops solutions that combine the efficiency of the free-market system with public policies that promote greater economic opportunity and outcomes for all.</p> 159 [empty string] <p>Regulation can provide a number of societal benefits such as improve food safety, ensures safety and efficacy of pharmaceuticals, protects consumers against fraud, promotes safety in transportation, protects workers in the workplace, and helps enforce civil rights laws in areas such as fair housing.  But regulation can also have real costs as well -- costs to business, costs to consumers, costs to American competitiveness in the world. Resources that could be spent on new investment instead go towards regulatory compliance, and the impact on smaller businesses can be particularly profound. The fundamental tool of regulation should be cost-benefit analysis.</p> <p>CED has promoted the idea of &ldquo;smart regulation&rdquo; – the idea that regulation should achieve the purpose for which it was imposed at the lowest possible cost and with the maximum possible benefits. Smart regulations are well-designed at the outset and must be continually reviewed, revised, or sunset, particularly given the rapid pace of technological change. perspectives about the role of money in politics and bipartisan reforms for policymakers to enact. <ol style="margin-top:10px"> <li>Conducted poll of Chicago business executives about influence and corruption in city policies and politics. </li> <li>Polled New Mexico business leaders about their perceptions of money in politics and proposals the New Mexico state Legislature may consider to change transparency laws and the campaign finance system. </li> </ol> </li> <li>Members speak at policy forums and to state and national lawmakers about the importance of reform to improve trust in our democratic institutions. </li> <li>Conducted an analysis of financial activity in the 2014 elections, as well as the 2016 cycle, from January 2015 through July 2016, to gain further insight into the behavior of corporations and business organizations in the wake of <em>Citizens United</em>. </li> <li>Explored the numerous ways in which government-private sector transactions have reduced economic efficiency at the expense of the public interest. </li> </ul> <h3><strong><em>CED Policy Recommendations</em></strong></h3> <p>CED has developed recommendations for both policy makers and business leaders to adopt to decrease the impact of Crony Capitalism:</p> <ul> <li>Business leaders must refrain from pursuing crony deals and become spokesmen for the importance of competition on a level playing field.< </li> <li>Business leaders must demand the same disclosure and transparency in our electoral system that they are required to offer in their firms&rsquo; finances. </li> <li>Policy makers should implement a matching system for small-dollar contributions for federal elections that will make it possible for our elected policymakers to run without reliance on big-dollar donors. </li> <li>More states should move from election-based to appointment-based judicial systems, to end the unseemly reliance of prospective judges on contributions from individuals and interests who may later come before the bench. </li> <li>Create a commission-based appointment process to hold judges accountable. Commissions should be independent and nonpartisan, and responsible for preparing a list of nominees for judicial appointments to be made by a state&rsquo;s governor. </li> <li>Develop judicial performance evaluation commissions. These independent and nonpartisan entities would be responsible for recommending whether a judge should be retained in office. 25 [empty string] <p>CED has long viewed US fiscal health as crucial to economic growth and business operations. Our national debt continues to grow unchecked, now exceeding $36 trillion. The cost of servicing this debt, now more than the US spends on defense, cripples our ability to adapt government spending to current needs. A national debt-to-GDP ratio of 100%, estimated to reach 180% by 2050, is already unsustainable and will inevitably crush any future ambitions we hold for our nation’s potential.</p><p>CED believes it is essential for US economic growth and national security that the Administration and Congress take measures to halt and eventually reverse the growth of the national debt to a more sustainable 70% of GDP. This can only be achieved through policies that promote productivity and through difficult choices in budgeting, including tax reforms and expense reductions. Additionally, the principal Social Security Trust Fund will become depleted in a decade without action and Medicare faces the reality of needed modernization. </p><p>To support lawmakers and the public in facing these challenges, CED serves as a resource for the business perspective on these important fiscal policy issues. This balance is best achieved through a market-driven system, based on private-sector competition and cost-conscious consumer choice.</p> 158 [empty string] <p>Health care is simultaneously a deeply personal issue and a more mundanely economic one. The personal consequences are known to all of us. Economically, many households fear the financial risk of a prolonged illness or serious injury. Health care costs are the most frequent cause of personal bankruptcy. And, from a national perspective, growing health care costs have profound implications for the country&rsquo;s long-term fiscal health.</p> <p>Because of health care&rsquo;s very central role in our personal and economic wellbeing, CED believes strongly that:</p> <ul> <li>Every American should have access to health care.</li> <li>Care should be of high quality.</li> <li>Care should be affordable.</li> </ul> <p>The Patient Protection and Affordable Care Act of 2010 (known informally as the ACA) was a well-intentioned attempt to deal with these issues, but in CED&rsquo;s view, it has fallen short. Further reform is necessary to retain the ACA&rsquo;s positive elements, while simultaneously improving upon it to build a better health care system for all Americans.</p> <p>Our vision for Health Care builds on the ACA&rsquo;s advances by strengthening and broadening the new law&rsquo;s use of those market incentives to drive innovation for higher quality and lower cost, while maintaining an appropriate role for government to facilitate access and ensure that markets work. We believe that this truly would be the achievement of all three objectives of quality, affordability, and access that policymakers have sought for many years. </p> <p>CED believes policymakers have an opportunity to transform the ACA into a system driven by market incentives that decrease costs, improve quality, and promote innovation—while increasing access to coverage. Our research also explores the best path forward for Medicare, which is projected to be the strongest cost driver in the federal budget.</p> <p>CED Members play an instrumental role in forming CED research findings, policy statements and recommendations for reform. Members take an active role in raising awareness about the need for reform through local forums, op-eds, and media appearances. Our members and staff routinely briefs lawmakers about how its recommendations benefit patients, providers, and government.</p><h3><strong><em>CED Policy Recommendations</em></strong></h3> <p>CED has developed recommendations that harness market forces under both the Affordable Care Act and Medicare. </p> <p>Affordable Care Act:</p> <ul> <li>Replace the ACA&rsquo;s complex subsidy mechanism, which puts a heavy compliance burden on and may mislead families with modest incomes and has proved difficult to administer accurately. </li> <li>Restructure the ACA exchange system to align more closely with cohesive geographic health care market areas.</li> <li>Broaden the exchange populations to increase the numbers of enrollees and also the risk diversity, especially in small geographic areas.</li> <li>We would expand the ACA&rsquo;s increase in consumer choice of insurance plans—which is the key to competition and innovation. </li> <li>Remove the ACA&rsquo;s unpopular mandates—and their complex exemptions—to compel the purchase of insurance.</li> <li>Replace the ACA&rsquo;s income-conditioned premium subsidies with a &ldquo;fixed-dollar&rdquo; refundable tax credit, usable only to purchase insurance. </li> <li>Eliminate the unnecessary individual and employer mandates. </li> <li>Reform the tort system, using new data and analysis to formulate rebuttable standards of sound practice. Create specialized expert courts to facilitate more timely and less costly decisions.</li> </ul> <p>Medicare:</p> <ul> <li>Eliminate the Medicare Advantage price benchmark based on traditional Medicare&rsquo;s fee-for-service cost, and provide enrollees with a premium subsidy. </li> <li>Increase the income-conditioning of Part B and Part D premiums, including a temporary Part B premium reduction for lower-middle income seniors. </li> <li>Risk-adjust premium revenue for plans. </li> <li>In rural areas, allow Medicare beneficiaries to enroll in traditional Medicare at no additional out-of-pocket cost, until Medicare Advantage plans meet a minimum threshold of availability.</li> </ul> [empty string] Health Care To address rising health care costs, low-quality care, and inadequate access, CED advances recommendations for market-based and consumer-driven health care reform. To put capitalism on a sustainable path, CED develops solutions that combine the efficiency of the free-market system with public policies that promote greater economic opportunity and outcomes for all.</p> 159 [empty string] <p>Regulation can provide a number of societal benefits such as improve food safety, ensures safety and efficacy of pharmaceuticals, protects consumers against fraud, promotes safety in transportation, protects workers in the workplace, and helps enforce civil rights laws in areas such as fair housing.  But regulation can also have real costs as well -- costs to business, costs to consumers, costs to American competitiveness in the world. Resources that could be spent on new investment instead go towards regulatory compliance, and the impact on smaller businesses can be particularly profound. The fundamental tool of regulation should be cost-benefit analysis.</p> <p>CED has promoted the idea of &ldquo;smart regulation&rdquo; – the idea that regulation should achieve the purpose for which it was imposed at the lowest possible cost and with the maximum possible benefits. Smart regulations are well-designed at the outset and must be continually reviewed, revised, or sunset, particularly given the rapid pace of technological change. CED is deeply concerned about challenges to the strength of the nation’s democratic institutions, including practices that elevate the interests of private actors at the expense of the public good. To ensure the health and vitality of our democracy, CED continues to engage in research and outreach focused on restoring the strength of, and trust in, a government that is responsive and accountable to the people.</p> 161 [empty string] <p>CED believes that the success of American democracy depends on an engaged electorate, a transparent campaign finance system, and on fair and impartial courts.  Since 2000, CED has conducted extensive research and created policy solutions that address the role of hidden money, foster greater citizen participation in elections, and insulate state courts from political influence.</p> <h3><em><strong>Election 2020</strong></em></h3> <p>The Committee for Economic Development of The Conference Board (CED) strongly believes that transparent, fair, and accessible elections are fundamental to the confidence of US citizens in their government and its leadership, and to the long-term health of the US economy.</p> <p>To protect voters and poll workers and maintain trust in the conduct and outcome of elections, policymakers and election officials should adopt election best practices designed to address the COVID-19 pandemic, including:</p> <ul> <li>Choosing appropriate polling sites and adjusting the polling-place environment and procedures for safe in-person voting</li> <li>Providing appropriate staffing and equipment for polling sites and training for poll workers</li> <li>Offering at least 10 days of early voting, including some weekend and evening hours</li> <li>Offering postage-paid, &ldquo;no-excuse&rdquo; absentee voting, supported by: <ol style="margin-top:10px"> <li>Voter registration and absentee ballot requests without in-person trips to election offices or notaries, including postage-paid applications for absentee ballots mailed to all registered voters where feasible</li> <li>Early processing or counting of absentee ballots received prior to Election Day, with clear communication of the status of absentee ballot submissions</li> <li>Secure, video-monitored absentee-ballot drop boxes with tight chains-of-custody procedures</li> <li>Best practices for absentee ballot design to avoid missing or misplaced signatures</li> <li>Clearly communicated absentee ballot deadlines, based on when a ballot is posted rather than when it is received, backed by a commitment from the US Postal Service to postmark all absentee ballots received</li> </ol> </li> </ul> <p>Business leaders should take concrete steps to assist in the election effort—helping to protect their employees, customers, and communities—including:</p> <ul> <li>Communicating new voting rules, options for voting, and voting procedures to employees and customers, including providing reminders to help their own employees vote early</li> <li>Directly aiding election officials through the donation of needed infrastructure, technology assistance, supplies, and services</li> <li>Making it easier or more affordable for employees to serve as poll workers</li> <li>Providing paid leave to facilitate safe voting and reduced crowding at the polls</li> </ul> <h3><em><strong>Money &amp; Politics</strong></em></h3> <p>In recent years, CED has conducted the following activities to address the corrupting influence of Money in Politics:</p> <ul> <li>Through state and national polling, CED surveyed business leaders&rsquo; perspectives about the role of money in politics and bipartisan reforms for policymakers to enact. <ol style="margin-top:10px"> <li>Conducted poll of Chicago business executives about influence and corruption in city policies and politics. </li> <li>Polled New Mexico business leaders about their perceptions of money in politics and proposals the New Mexico state Legislature may consider to change transparency laws and the campaign finance system. </li> </ol> </li> <li>Members speak at policy forums and to state and national lawmakers about the importance of reform to improve trust in our democratic institutions. </li> <li>Conducted an analysis of financial activity in the 2014 elections, as well as the 2016 cycle, from January 2015 through July 2016, to gain further insight into the behavior of corporations and business organizations in the wake of <em>Citizens United</em>. </li> <li>Explored the numerous ways in which government-private sector transactions have reduced economic efficiency at the expense of the public interest. </li> </ul> <h3><strong><em>CED Policy Recommendations</em></strong></h3> <p>CED has developed recommendations for both policy makers and business leaders to adopt to decrease the impact of Crony Capitalism:</p> <ul> <li>Business leaders must refrain from pursuing crony deals and become spokesmen for the importance of competition on a level playing field.< </li> <li>Business leaders must demand the same disclosure and transparency in our electoral system that they are required to offer in their firms&rsquo; finances. </li> <li>Policy makers should implement a matching system for small-dollar contributions for federal elections that will make it possible for our elected policymakers to run without reliance on big-dollar donors. </li> <li>More states should move from election-based to appointment-based judicial systems, to end the unseemly reliance of prospective judges on contributions from individuals and interests who may later come before the bench. </li> <li>Create a commission-based appointment process to hold judges accountable. Commissions should be independent and nonpartisan, and responsible for preparing a list of nominees for judicial appointments to be made by a state&rsquo;s governor. </li> <li>Develop judicial performance evaluation commissions. These independent and nonpartisan entities would be responsible for recommending whether a judge should be retained in office. 20 [empty string] <p>CED has long viewed US fiscal health as crucial to economic growth and business operations. Our national debt continues to grow unchecked, now exceeding $36 trillion. The cost of servicing this debt, now more than the US spends on defense, cripples our ability to adapt government spending to current needs. A national debt-to-GDP ratio of 100%, estimated to reach 180% by 2050, is already unsustainable and will inevitably crush any future ambitions we hold for our nation’s potential.</p><p>CED believes it is essential for US economic growth and national security that the Administration and Congress take measures to halt and eventually reverse the growth of the national debt to a more sustainable 70% of GDP. This can only be achieved through policies that promote productivity and through difficult choices in budgeting, including tax reforms and expense reductions.
