Evan Richardson founded Form Health in 2019. A healthcare technology pioneer, Evan previously helped build Castlight and was a founding team member of Grand Rounds (now Included Health). He began his career at Bain & Company and Berkshire Partners and serves on the boards of Bicycle Health and Crossover Health. Evan holds a computer science degree from Dartmouth and an MBA from Harvard Business School.
